"assertion failure" at startup???

On OmniWeb 4.0.3, I've started to notice in the Error Log some weird
messages when I start it up (it's set to load up omniweb:/StartPage/ and
not any online home page).

Aug 03 16:12:54  *** Assertion failure in -[NSMenu itemAtIndex:], NSMenu.m:552
Aug 03 16:12:54  Invalid parameter not satisfying: (index >= 0) && (index < [_itemArray count])

I also see more problems with the web browser hanging right after a page
request (only quiting/restarting fixes this), but I don't know if this
is to the error messages above or not.
