questions about printing

Apologies if this has been covered in the past...the mailing list search function isn't working. I've used OO for some time but oddly haven't printed outlines that frequently. Now that I'm trying to print, I've got some questions.

What determines  the width of a column on the printed page? I have 4 columns but am only printing the first, which has a lot of text in it. I can expand the column to the "page width" line, which does fill my page width, but it's not WYSIWYG--the printout doesn't exactly match the line endings on the screen. I'm just curious about this.

Also, any way to format the font of the header and footer?

Finally, any way to prevent, for example, a level one row printing all by itself on the bottom of a page?
