Page Setup usability problem

Hi Rachel-

Did you possibly cancel out of the 'Don't Customize First Page'  
option? When you select it, you should get a dialog box that asks if  
you're sure you want to remove the custom header/footer for the first  
page. If you push remove, the list will now say "All Pages" and in  
place of "Don't Customize First Page" it will say "Customize First  
Page".
