OmniOutliner doesn't support this sort of dynamic update.  A template  
merely provides a starting point for a document.  Once a document is  
created from a particularly template it is no longer associated with  
that template in any way.

As Nicholas suggested in his reply, OO isn't a great tool for  
creating forms.
