Can't add item to a 'naked' outline

You can't add a new item to an outline in which you've deleted all the 
items unless you happen to have activated the 'add item'  toolbar icon. 
There's no 'add item' menu item (and no  contextual menu item for 'add 
item').

The use of an outline in drafting documents, to me, is that I can 'start 
anywhere' with some thoughts and then add organization and direction as 
I go.

It just turns out that up to now I've never been so unhappy with an 
outline that I've wanted to delete the whole thing and start again.

Just as well.... because you can't. Unless you have 'add item' in the 
toolbar. I didn't.
