When you change the password via System Preferences, the keychain
password is be updated too. If you change it with "Password Reset" -
an application on your install DVD - then the keychain keeps the old
password. Same goes for password changes with passwd in a shell.

In case the user doesn't know his/her old password, I don't see any
other way.
